To help you better understand back pain, we invite you to take an educational online assessment. When you answer a few quick questions, you’ll receive informational tips that may help you relieve back pain and prevent it in the future. This assessment is voluntary and your results are not tracked or shared with anyone. Remember, you should always discuss any symptoms with your doctor.

  • Provider – Select a quality provider for a procedure. Research available network providers for specific procedures. See the quality and efficiency of these providers based on the UnitedHealth Premium Tier 1 program.
  • Price – Quickly estimate out-of-pocket costs for specific procedures. Compare estimated prices of treatment options. Evaluate opportunities to reduce out-of-pocket costs.
  • Place - Locate providers based on geographic search criteria. Get addresses and directions to selected locations.
