Electronic W-2, 1095-C consent deadline Jan. 12

The last day to consent to receive 2017 W-2 and 1095-C forms electronically from UHD is Friday, January 12 at 5 p.m.

To make  electronic W-2/1095-C designations in PeopleSoft/P.A.S.S follow the instructions listed below. An automated email will be sent to  once submission and consent occurs.​

Benefits of consenting to electronic W-2 and 1095-C forms. 

  • Earlier access to the W-2 and 1095-C forms than paper copies.
  • No possibility for lost, stolen, delayed or misplaced forms.
  • Access from any Internet connection at any time via P.A.S.S.
  • Employees have the option to print multiple copies at their convenience.
  • Once consent is given, it remains in force.  No annual renewal is required.

If electronic W-2 and 1095-C forms are chosen, a hard copy of each form will be mailed to the employee’s address on file in PeopleSoft via USPS no later than January 31, 2018.

Electronic W-2 Instructions

  1. Log in to P.A.S.S. to consent to receive W-2 form electronically. Once in P.A.S.S., go to “Employee Self Service” (top of the screen). If  a manager, P.A.S.S. automatically comes up under “Manager Self Service.” By clicking the down arrow next to “Manager Self Service,” it will switch to “Employee Self Service.”

 

  1. Click the “Payroll and Compensation” link.

 

  1. Click the “W-2/W-2c Consent” link.

 

  1. Check the “I consent to receive W-2 or W-2c forms electronically” box and click “Submit.” If  currently set up to receive W-2 or W-2c forms electronically, as per below, don’t  do anything else. By checking the “I withdraw my consent to receive W-2 or W-2c forms electronically” box, consent will be withdrawn.

 

Electronic 1095-C Instructions

  1. Log in to P.A.S.S. to consent to receive 1095-C form electronically. Once in P.A.S.S., go to “Employee Self Service” (top of the screen). If a manager, P.A.S.S. automatically comes up under “Manager Self Service.” By clicking the down arrow next to “Manager Self Service,” it will switch to “Employee Self Service.”

 

  1. Click the “Benefits Summary” link.

 

  1. Click the “Form 1095-C Consent” link.

 

  1. Check the “I consent to receive Form 1095-C electronically” box and click “Submit.” If currently set up to receive Form 1095-C electronically, as per below, don’t do anything else. By checking the “I withdraw my consent to receive Form 1095-C electronically” box, consent will be withdrawn.

 

What else must I do?

While in P.A.S.S., validate the following information is correct:

  • Name
  • Social Security Number
  • Mailing Address

IRS Regulations require the name on the W-2 match the name listed on the Social Security Card. If  PeopleSoft name doesn’t match the name listed on the Social Security Card, bring a copy of Social Security Card to ESO (910-S) so the required changes can be made.
