The care management program gives HealthSelectSM plan participants access to a team of dedicated clinicians for personalized care and support to help manage physical and mental health conditions.

Employees and dependents in HealthSelect of Texas® and Consumer Directed HealthSelectSM can access the Blue Cross and Blue Shield of Texas program for help managing chronic conditions, understanding a new diagnosis or just navigating the health care system. 

A new diagnosis can be scary, and you may not know what your next steps are or whom to contact with questions. If you are enrolled in HealthSelect of Texas or Consumer Directed HealthSelect, Blue Cross and Blue Shield of Texas’ (BCBSTX’s) care management program gives you support and personalized care. BCBSTX offers the program as a part of your HealthSelect plan.

At the heart of the care management program is a team of Texas-based clinicians who can give you personalized help and support for all types of health conditions.  A care management clinician may contact you if you have been diagnosed with or treated for certain health conditions. Care management is available at no out-of-pocket costs for you. 

Call a care management clinician for help with managing many physical or mental health conditions, from asthma to heart disease, anxiety or depression—and more. You can get information about a new diagnosis and assistance with finding health and wellness resources.
