As a HealthSelectSM participant, you have many resources that can help you improve your health and wellness. One of the most useful tools, no matter your level of health and wellness knowledge, is the Well onTarget® Health Assessment.
Well onTarget is an online wellness portal that offers personalized resources and incentives to support your wellness journey. To access Well onTarget in Blue Access for MembersSM, click “Well onTarget” under the Quick Links on the left.
The first time you log into your Well onTarget account, you’ll be asked to complete a Health Assessment.
The Health Assessment takes about 15 minutes to complete. It asks several confidential questions about your lifestyle and health habits to provide an overall assessment of your current health, including how your health compares to others in your age group. It identifies what you are already doing well—and shows where there is room for improvement. The Health Assessment adapts questions and suggestions based on your input and provides evidence-based action steps to assist in changing behaviors.
While it’s not required, you might find it helpful to get a wellness exam with your primary care provider before taking the assessment. That way, you will have the health information you need, like lab work results, to answer assessment questions more accurately.
When you complete your assessment, your Well onTarget portal will be tailored to your specific needs, based on your responses. Your dashboard will display relevant programs that may help you reach your goals.
You will also get a Personal Wellness Report, which includes:
- information about health risks,
- wellness recommendations,
- preventive care recommendations,
- suggestions for next steps in your health and wellness journey and
- recommendations for other health and wellness resources.
You can also compare your results with previous reports if you’ve taken the Health Assessment before.
When you complete your Health Assessment, you get a certificate (it will not contain any personal health information). Check with your agency’s benefits coordinator to see if your employer gives wellness incentives for completing the Health Assessment. Just print or download the certificate and give it to the appropriate person at your agency.
To access your certificate, go to the left-hand menu on the top of the Well onTarget website, and click on “Certificates.” All prior certificates are listed here as well.
You can take the assessment twice a year: once in the first half of the calendar year and once in the second half of the calendar year. Each time you complete the Health Assessment, you’ll earn 2,500 Blue PointsSM, which you can redeem for a wide variety of items including fitness gear, wearables and camping equipment. You’ll get a reminder when you log in to the portal whenever you’re eligible to complete a new assessment.
