HealthSelect Self-management programs help participants meet their wellness goals

HealthSelect of Texas® and Consumer Directed HealthSelectSM participants have access to tools and resources to help them reach their wellness goals. Well onTarget® offers participants a personalized plan.

Here’s how:

  • When you first open Well onTarget, please complete a brief online Health Assessment. The Health Assessment will identify what you are already doing well and where there are opportunities for improvement.
  • After you take the Health Assessment, you will get a personal wellness report with recommended goals and action steps based on your responses. What’s more, you will earn Blue PointsSM for taking your Health Assessment and participating in Well onTarget wellness activities.

There are 26 digital, self-management programs participants can select through Well onTarget to support their health journeys. If a participant has taken the Health Assessment, programs will be suggested based on their responses to the questionnaire.

The programs provide a structured set of activities, support resources, as well as clinical and behavioral information specific to a participant’s needs.

Some courses, like Quitting Tobacco, Weight Management and Managing Stress, are designed to help participants improve their health. Others, like Preventive Health and Sleep Health, are meant to help participants maintain healthy lifestyles.

You can access Well onTarget through Blue Access for MembersSM or through the AlwaysOn® Wellness smartphone app, which is available on Google Play and the App Store.

Resources help keep you on track 

In addition to self-management programs, Well onTarget offers a variety of other wellness resources, including a symptom checker, personalized wellness coaching and health trackers that can sync to most fitness devices.
