UHD staff and faculty that missed the September Flu Shot Clinic get a second chance this Tuesday to take advantage of the convenience of getting the immunization while at work.

All employees that participate in the HealthSelect of Texas medical insurance plan can stop by the UHD Special Events Center (A-300) between 3 and 6 p.m. on Tuesday, October 21 to get their no cost flu shot.

Flu shots are available on a first come, first served basis. The clinic will end prior to 6 p.m. if the administrator runs out of vaccines. In order to ensure enough supplies are brought, please register here.

In addition to registering online, make sure to:

1. Print AND fill out a completed Vaccine Administration Record (VAR) Consent form.
2. Hand write your medical insurance Member ID and Group Number somewhere on the back of the VAR form
3. Bring the completed Vaccine Administration Record (VAR) Consent form AND your UnitedHealthcare HealthSelect insurance card with you on October 21.

The vaccine administered at the clinic is the Fluvirin vaccine.
